To personalize your plaque with a special message, indicate in the space to the right how you would like your pet's plaque to read. AMS will send you a confirming email within 24 hours to verify your order and your personal message.
IMPORTANT - AMS will select the best plaque size for your inscription based upon your input. The largest plaque is 3" x 4" so there is a limit to the space available. If you have any comments please place them below what you would like AMS to inscribe on the plaque.
